A man in his late 20s presented with a 3-year history of a burning, pruritic, scaly rash on his face, trunk, and extremities that had substantially worsened within the previous 6 months. He reported weight loss, arthralgias, substantial proximal muscle weakness, and weakness of his voice that started approximately 1 year prior. He formerly operated machinery but had to change to a less physically demanding job. At the time of presentation, previous treatments included hydroxychloroquine, 200 mg, twice daily; methotrexate, 20 mg, weekly; folic acid, 1 mg, daily; and topical triamcinolone. He reported regular use of sunscreen and wearing UV protective sleeves.
