A patient in their 50s arrived at the cardiac clinic concerned about chest tightness and dyspnea for 2 months since recovering from COVID-19. There was no history of hypertension, hypertrophic cardiomyopathy, or diabetes; family medical history was unremarkable. Physical examination showed a heart rate of 118 beats per minute (bpm) and blood pressure of 105/70 mm Hg. Laboratory values were: potassium, 4.02 mEq/L; troponin I, 0.18 ng/mL; creatine kinase-MB fraction, 16 IU; N-terminal pro-B–type natriuretic peptide, 2377 pg/mL, and D-dimer 0.39 mg/L (to convert to nmol/L, multiply by 5.476). An electrocardiogram (ECG) was obtained (Figure, A).
