A 72-year-old man presented to the emergency department with 3 days of fevers, night sweats, and rash. He reported condomless vaginal intercourse with a new partner 20 days prior to presentation. On physical examination, his temperature was 99.7 °F (37.6 °C) and he had bilateral inguinal, axillary, and cervical lymphadenopathy and a maculopapular rash on his palms and soles. Laboratory testing results showed a white blood cell count of 1690/?L, platelet count of 37,000/?L, and a rapid plasma reagin titer of 1:1024 (reference, nonreactive). The result of a fourth-generation HIV-1/2 antigen-antibody immunoassay (fourth-generation HIV test) was positive. The result of HIV-1-/2 antibody differentiation testing was negative.
