Ambulatory surgeries performed at ambulatory surgical centers (ASCs) rather than hospital-based outpatient departments (HOPDs) are safe and cost-effective and associated with increased patient satisfaction.- In July 2020, to increase appropriate use of the health system’s ASCs, our institution implemented a process in which physician assistants within the preoperative clinic prospectively review case requests for the patient’s suitability for surgery at an ASC relative to institutional guidelines. These reviews are intended to increase appropriate use of ASCs by reducing late cancellations of surgeries scheduled to be performed at an ASC as well as by identifying candidates for surgery at an ASC among cases requested for the HOPD. The manual review is accurate and has improved appropriate ASC use, but it is labor intensive, especially in the context of ongoing nationwide clinical staffing challenges.
