The emergence of immune checkpoint inhibitors has reshaped the treatment landscape for cutaneous squamous cell carcinoma (cSCC), introducing new opportunities—and complexities—around when and how to integrate immunotherapy with surgery and radiation. Despite growing evidence supporting neoadjuvant and adjuvant approaches, clinicians continue to face uncertainty regarding patient selection, timing of specialist involvement, and how to counsel patients through decisions that may include delaying or foregoing surgery. This online, on-demand CME/CE activity—part of the six-part series Mock Tumor Board: Advancing Multidisciplinary Decision-Making in Cutaneous Squamous Cell Carcinoma—will apply evidence-based criteria to guide the selection and sequencing of immunotherapy with surgery and radiation, particularly in the perioperative setting. Expert faculty will revisit risk-stratification frameworks for recognizing high-risk features—including perineural invasion, nodal burden, and extracapsular extension—that guide management of locally advanced, recurrent, nodal, and metastatic cSCC. They will also discuss how to properly communicate treatment expectations and potential toxicities of immunotherapy to patients for therapy.
