Course: Woman in Her 50s With Pulmonary Hypertension Associated With Takayasu Arteritis

A woman aged 55 years was referred to the hospital due to a 2-month history of dyspnea. Right heart catheterization showed a mean pulmonary artery (PA) pressure of 57 mm Hg. When contrast-enhanced computed tomography (CT) showed thickening and contrast enhancement in the wall of the aortic arch, Takayasu arteritis was suspected. Fluorine 18 (18F)–fluorodeoxyglucose (FDG) positron emission tomography (PET)/CT showed high 18F-FDG uptake in the PAs (, A). A diagnosis of Takayasu arteritis was confirmed, and vasculitis in the PAs was presumed to be the cause of pulmonary hypertension (PH). After receiving PH-specific and immunosuppressive therapy, lower 18F-FDG uptake was observed in the PAs on PET/CT (, B), and the patient’s mean PA pressure decreased 21 mm Hg. PA involvement in Takayasu arteritis rarely induces PH.? 18F-FDG PET/CT presented useful information that was helpful for the initial diagnosis and evaluation of disease activity.-
