Job Responsibilities
  • Assess patient strengths, limitations, and needs using standardized measurement, prediction, and evaluation tests

  • Develop and implement individualized short‐ and long‐term treatment goals based on patient assessments

  • Administer and interpret clinical physical therapy services, adjusting treatment plans as needed based on patient response

  • Maintain accurate and timely patient treatment records, documenting progress and changes in electronic health systems

  • Ensure all documentation meets requirements of reimbursement and regulatory agencies, completed within two business days

  • Establish and maintain effective working relationships with patients, families, and interdisciplinary team members

  • Participate in patient care conferences and collaborate with other healthcare professionals for coordinated care

  • Provide education to patients and families regarding treatment plans, safety, and self‐care techniques

  • Participate in discharge planning and make appropriate referrals, coordinating with vendors for durable medical equipment

  • Supervise and provide direction to Physical Therapist Assistants during treatment sessions

  • Maintain licensure requirements and pursue continuing education to stay current with professional standards

  • Engage in departmental and organizational performance improvement initiatives to enhance quality of care

Position Qualifications

Minimum Required:

  • Possession of a current license or eligibility to be licensed as a Physical Therapist in the State of New York

  • Graduate of an accredited school of physical therapy with a Bachelor's or Master's Degree or equivalent in physical therapy

  • Current CPR certification

  • Previous experience in a licensed healthcare setting

Preferred:

  • Master's or Doctorate Degree in Physical Therapy

  • Previous physical therapy experience in a hospital‐based setting

Work Environment

  • Majority of work is performed in the patient home. Work in the office requires working in close quarters with other employees. The offices are air conditioned/heated. Gloves, masks, gowns, other personal protective equipment are used, as per blood and body fluid and isolation precautions.

  • Adolescent: 13 to 17 Years
  • Early Adulthood: 18 to 45 Years
  • Middle Adult: 46 to 59 Years
  • Late Adult: 60 to 79 Years
  • Elderly: 80+ Years
