Abstract:
Portal vein tumour thrombus (PVTT) occurs very frequently in patients with hepatocellular carcinoma (HCC) because the biological behaviour of HCC results in a strong likelihood of vascular invasion. PVTT incidence varies across countries and regions, and ranges from % to %. HCC patients with PVTT have poor prognosis, with a median survival time associated with the best supportive care of only to months., The American and ESMO guidelines for the diagnosis and treatment of HCC, which is based on the Barcelona Clinical Liver Cancer (BCLC) staging system, recommend systematic treatment regimens such as targeted drugs and immune drugs., However, guidelines for the diagnosis and treatment of HCC in Asia such as Asian-Pacific guidelines and guidelines from mainland China, Korea and Taiwan all indicate that local therapies (eg, hepatic resection, radiotherapy, TACE and HAIC) may be used as an optional regimen for patients with PVTT in addition to targeted drugs and immunotherapy as the first-line treatment.
